Background: Managing HIV treatment is a complex multi-dimensional task because of a combination of factors such as stigma and discrimination of some populations who frequently get infected with HIV. In addition, patient-provider encounters have become increasingly multicultural, making effective communication and provision of ethically sound care a challenge. Purpose: This article explores ethical issues that health service providers in the United States and Botswana encountered in their interaction with patients in HIV care. Research design: A descriptive qualitative design was used (...) to collect data from health service providers and patients using focused group discussions. This article is based on responses from health service providers only. Participants and context: This article is based on 11 focused group discussions with a total sample of 71 service providers in seven US sites and one Botswana site. Ethical considerations: Ethical review boards at all the study sites reviewed the study protocol and approved it. Ethical review boards of the study’s coordinating centers, Rutgers University and the University of California at San Francisco, also approved it. The study participants provided a written informed consent to participate. Findings: HIV service providers encountered ethical challenges in all the four Beauchamp and Childress’ biomedical ethics of respect for patients’ autonomy, beneficence, justice, and nonmaleficence. Discussion: The finding that HIV service providers encounter ethical challenges in their interaction with patients is supported by prior studies. The ethical challenges are particularly prominent in multicultural care and resource-constrained care environments. Conclusion: Provision of HIV care is fraught with ethical challenges that tend to pose different issues depending on a given care environment. It is important that strong partnerships are developed among key stakeholders in HIV care. In addition, health service providers need to be provided with resources so they can provide quality and ethically sound care. (shrink)

I wouldn't recommend this to the faint of heart. It's a bit terrifying. It's well written, but it tells the long and tangled tale of how theories of the ENSO phenomenon evolved from 1969 to 1998 — a period that saw much progress, but did not end with a neat, clean understanding of this phenomenon. It's packed with hundreds of references, and sprinkled with somewhat intimidating remarks like: The Fourier decomposed longitude and time dependence of these eigensolutions obey dispersion relations (...) familiar to every physical oceanographer... Nonetheless I found it fascinating — so, I'll pick off one small idea and explain it now. (shrink)

This paper argues that the agency/structure dichotomy thatpredominates in racism discourse is problematic because itobscures how racism is produced and resisted at the local sitesof relations between individuals and between individualsand institutions. Racism permeates social relations,ensured by `knowledge' and guaranteed through self-regulation. Resistance to racism requires arecognition of racism's `local' character. As aresult, educators, particularly in classrooms,play important roles in resistance-practices.

E. P. Wigner 's argument that the probability of the existence of self-reproducing units, e.g., organisms, is zero according to standard quantum theory is stated and analyzed. Theorems are presented which indicate that Wigner 's mathematical result in fact should not be interpreted as asserting the improbability of self-reproducing units.

The detrimental effect of human behavior on the environment is undeniable. Attitudes are recognized as a predictor of the pro-environmental behavior; therefore, having good quality tools in Colombia to measure them is strategic to assess interventions. This study aims to establish psychometric indicators for the pro-environmental attitudes questionnaire Colombian version to a sample of 415 volunteers aged 18–70 years. We used the 28-item PEAQ already linguistically adapted for Colombia. We applied the following questionnaires: Environmental awareness, environmental values, and the pro-environmental (...) at work questionnaire. We used a one-parameter Rasch model and Winsteps program to assess the PEAQ's one-dimensionality and item statistics by gender, and estimated Spearman's rho coefficient between the PEAQ scores and the scales for concurrent validity. The PEAQ in this study has 24 items because 4 items did not fit into the Rasch model criteria. Its one-dimensionality was supported by an explained variance and the first residual variance. The coefficients, α = 0.95 and Ω = 0.95; Rasch for persons = 0.90; and Rasch for items = 0.95. The correlation between the PEAQ and the EC, EV, and PEWQ scales were Spearman's rho coefficient = 0.859, 0.795, and 0.885, respectively. Thus, the PEAQ Colombian version's psychometric indicators support it as a valid and reliable instrument to measure pro-environmental attitudes in this country. (shrink)

The Distributional Compositional Categorical model is a mathematical framework that provides compositional semantics for meanings of natural language sentences. It consists of a computational procedure for constructing meanings of sentences, given their grammatical structure in terms of compositional type-logic, and given the empirically derived meanings of their words. For the particular case that the meaning of words is modelled within a distributional vector space model, its experimental predictions, derived from real large scale data, have outperformed other empirically validated methods that (...) could build vectors for a full sentence. This success can be attributed to a conceptually motivated mathematical underpinning, something which the other methods lack, by integrating qualitative compositional type-logic and quantitative modelling of meaning within a category-theoretic mathematical framework. The type-logic used in the DisCoCat model is Lambekʼs pregroup grammar. Pregroup types form a posetal compact closed category, which can be passed, in a functorial manner, on to the compact closed structure of vector spaces, linear maps and tensor product. The diagrammatic versions of the equational reasoning in compact closed categories can be interpreted as the flow of word meanings within sentences. Pregroups simplify Lambekʼs previous type-logic, the Lambek calculus. The latter and its extensions have been extensively used to formalise and reason about various linguistic phenomena. Hence, the apparent reliance of the DisCoCat on pregroups has been seen as a shortcoming. This paper addresses this concern, by pointing out that one may as well realise a functorial passage from the original type-logic of Lambek, a monoidal bi-closed category, to vector spaces, or to any other model of meaning organised within a monoidal bi-closed category. The corresponding string diagram calculus, due to Baez and Stay, now depicts the flow of word meanings, and also reflects the structure of the parse trees of the Lambek calculus. (shrink)

This article explores the relationships between legal proof and fundamental epistemic concepts such as knowledge and justification. A survey of the legal literature reveals a confusing array of seemingly inconsistent proposals and presuppositions regarding these relationships. This article makes two contributions. First, it reconciles a number of apparent inconsistencies and tensions in accounts of the epistemology of legal proof. Second, it argues that there is a deeper connection between knowledge and legal proof than is typically argued for or presupposed in (...) the legal literature. This connection is illustrated through a discussion of the Gettier problem in epistemology. It is argued that the gap or disconnect between truth and justification that undermines knowledge in Gettier cases also potentially undermines the success of legal verdicts. (shrink)

Questions of patient autonomy have formed an important part of ethical debate in medicine from at least the post-war period onwards. Although initially important as a counterweight to widespread medical paternalism, recent years have seen a reaction against a widely perceived ‘triumph of autonomy’. In particular, competent patients' refusal of life-saving or clearly beneficial treatment presents complex dilemmas for both healthcare professionals and ethicists. Discussion of the mechanism provided by the Israel Patient's Rights Act of 1996 for ethics committees to (...) override refusal of treatment by such patients has provided several examples in which it seems an obligation to benefit the patient can outweigh concerns regarding respect for their autonomy. However, such analyses as have been undertaken so far may be seen to omit important elements of the moral decision-making process that takes place even when the latter appears to ‘win out’. In addition to helping better prepare clinicians to meet such challenges in the future, a fuller appreciation of those elements that are present in justified cases of coercive treatment may enable them to arrive at a better understanding of what autonomy itself can mean in a medical context. In recent years, many non-consequentialists such as Frances Kamm and Thomas Scanlon have been puzzling over what has come to be known as the Number Problem, which is how to show that the greater number in a rescue situation should be saved without aggregating the claims of the many, a typical kind of consequentialist move that seems to violate the separateness of persons. In this article, I argue that these non-consequentialists may be making the task more difficult than necessary, because (...) allowing aggregation does not prevent one from being a non-consequentialist. The early Schelling and the romantics constructed the unconscious in order to overcome the modern split between subjectivity and nature, mind and body, a split legislated by Cartesian representationalism. Influenced by Boehme and Kabbalah, the later Schelling modified his notion of the unconscious to include the decision to be oneself, which must sink beneath consciousness so that it might serve as the ground of one's creative and personal acts. Slavoj Zizek has read the later Schelling's unconscious as a prototype of (...) Lacan's reactive unconscious, an unconscious that only exists as the excluded other of consciousness. This reading, though close to the text of Schelling, misses something essential: the unconscious for Schelling is not a repression but a condition of the possibility of life and love. (shrink)

BACKGROUND: Hallucinations and delusions that complicate Parkinson’s disease could lead to nursing home placement and are linked to increased mortality. Cognitive impairments are typically associated with the presence of hallucinations but there are no data regarding whether such a relationship exists with delusions. OBJECTIVE: We hypothesized that hallucinations would be associated with executive and visuospatial disturbance. An exploratory examination of cognitive correlates of delusions was also completed to address the question of whether they differ from hallucinations. METHODS: 144 PD subjects (...) completed a neuropsychological battery to assess cognition and the SAPS to examine psychosis. Correlational analyses assessed associations between hallucinations and delusions with cognitive domains. RESULTS: 48 subjects reported psychotic symptoms: 25 experienced hallucinations without delusions, 23 had symptoms dominated by delusions. Severity and/or number of hallucination subtypes were significantly correlated with lower scores in language, memory, attention, executive functioning, and visuospatial ability. Correlations with delusions were non-significant. Tests of differences in the size of the correlations between groups revealed a significant relationship between language and visuospatial performance with hallucinations. CONCLUSIONS: Cognitive correlates of hallucinations and delusions appear to be different in PD, suggesting distinct pathogenic mechanisms and possibly anatomical substrates. Hence, delusions may not share the same associations with dementia as hallucinations. Since this is a new finding, further studies will be needed to confirm our results. (shrink)

Consideration of the work of Sade in relation to Adorno usually refers to the much-discussed chapter from Dialektik der Aufklärung. But Adorno made a number of other remarks across his career that suggest a very different reading. I will discuss the three most significant of these remarks and show how they develop an approach to the libidinal aspect of aesthetic experience that challenges our understanding of the relation of thought and language. In doing so, Sade’s works indicate an extraordinary liberation (...) of the dialectic of natural-history through a mimesis of desire, which is made apparent in what Sade’s texts literally present; in their contingency and also their facticity.
